Case Study 1: INCIS – New Zealand Police:
In 1991 New Zealand Police embarked on a major IT project to provide a centralised computer system to provide information, investigation and analysis capabilities to the operational facilities, replacing a legacy system called LES. The prime contractor was IBM. The project was never fully completed, and ran over budget. To this day, the ramifications of this failed project are still felt within New Zealand Government, as every IT project over a certain size is closely watched to ensure the mistakes made during the INCIS project are not repeated

Case Study 2: NHS IT Project (UK) to Digitise Patient Records
This example failed IT project is from the UK. In 2002, the (at the time) UK Labour Government embarked on the largest Public Sector IT project ever. This was to digitise all the patient records for the UK’s National Health Service (NHS), to allow doctor’s to access any patient’s records anywhere. A worthy project, but a complete disaster. The failed IT project was eventually dismantled in 2011 by the coalition Conservative & Liberal government, costing the UK tax payer an estimated 12 billion pounds (!!).

Case Study 3: Victoria State Government, Australia – 10 years of Failed IT Projects
The federal state of Victoria in Australia, has started an internal “review” of 10 years of failed internal IT projects. The Victoria State ombudsman handed down a damning assessment of these projects, focusing on the poor IT project governance, and lack of leadership.
